- APD Medical Group Home – $10,000 (A licensed residential home that provides 24-hour care, medical oversight, and daily living support for individuals with developmental disabilities enrolled in the APD Medicaid Waiver program.
- APD Adult Day Training Center – $6500 (A supervised daytime program for adults with developmental disabilities that helps them build life skills, social skills, and work readiness while participating in structured community and learning activities.)
- Homemaker and Companion— $2,500 (A homemaker/companion provides for the maintenance of a safe and clean environment. Performs various housekeeping tasks, meal preparation and companionship activities for clients in their residences.)
Home Health & Skilled Services
- Home Health Services Provider — $3,000 (Billing Insurance Companies Directly for Services
Rendered – Sunshine Health, WellCare, Humana, etc.) - RN/LPN Home Health Provider — $3,000 (Billing Insurance Companies Directly for Services
Rendered using Self Nursing License) - Nurse Registry — $6,500 (An agency that offers health care related contracts for nurses, home
health aides, certified nursing assistants, homemakers, and companions in a patient’s home and as temporary staff to health care facilities) - Home Health Agency – $7500 (An agency that provides skilled services (by nurses, therapists, social
workers) and/or unskilled services by home health aides, certified nursing assistants, homemaker, companions) to patients in their homes.)
Transportation Provider
- NEMT Transportation Provider — $3,000 (Medicaid reimburses for medically necessary non-emergency transportation services for a Medicaid eligible recipient and a personal care attendant or escort, if required, who have no other means of transportation available to any Medicaid compensable service)
